Valley-Wide, SLVBHG open new clinic

VALLEY — Valley-Wide Health Systems, Inc. (Valley-Wide) is pleased to announce the opening of the new Alamosa South Health Center, located on the campus of the San Luis Valley Behavioral Health Group on County Road 9 South in Alamosa on October 12. 

Valley-Wide and the San Luis Valley Behavioral Health Group have worked closely together to bridge the physical health and the behavioral health services they provide. This integration project to improve access and effectiveness of healthcare services in the San Luis Valley began in 2009 with co-located services in all Valley-Wide clinics. They are excited to expand integrated services through a new model at the Alamosa South Health Center. 

The core of this approach is a behavioral health consultant working with a primary care provider in a system that supports team-based care. It will provide care coordination and support to individuals in meeting their self-management goals. The behavioral health consultant works with a primary care provider to evaluate the mind-body-behavior connection and provide brief, solution-focused interventions. 

Kyle Phillips FNP-BC and his team will be providing care on Tuesdays, Wednesdays and Thursdays with hours from 8 a.m. until 5 p.m. (closed from 12-1 p.m. for lunch).  As a Nurse Practitioner, Kyle has worked in internal medicine caring for the adolescent, adult and geriatric populations. In addition to this, he has worked in settings providing care to population-focused groups to provide primary care and urgent care services. Kyle has worked at Sierra Blanca Medical Center with Valley-Wide since 2015. His patients are welcome to see him at whichever site is most convenient for them.  He is looking forward to seeing many new patients as well.
