VALLEY — South Central Colorado Seniors in partnership with San Luis Valley Regional Medical Center will be hosting Chronic Disease Self-Management workshops in the San Luis Valley to help people with chronic diseases, their caregivers, and family members learn new skills and renew hope in their ability to manage their chronic disease.
Using a curriculum developed at Stanford University in 1991, the Chronic Disease Self-Management workshops will teach participants how to set goals and create action plans that include nutrition and physical activity. Offered at no cost, the workshops are led by peers and encourage interaction and mutual support, building participants’ confidence.
Beginning on Monday, May 20th thru June 24th, South Central Colorado Seniors will be offering Chronic Disease Self-Management classes in Del Norte at the High Valley Community Center. These free classes are six weeks long, every Monday from 6-8:30 p.m.
Classes will also be offered Wednesday, May 22 thru June 26th, at the SLV Education Center (formerly the Grizzly Inn Restaurant), Alamosa, for six weeks, every Wednesday from 6-8:30 p.m.
